Weight lossMounjaro

Mounjaro. Low to start, once a week.

A weekly tirzepatide pen that acts on two appetite hormones, GLP-1 and GIP. It’s approved in Japan for type 2 diabetes, so for weight loss it’s off-label use. A doctor decides whether it suits you.

How it works

It works with your appetite.

Tirzepatide acts like two gut hormones, GLP-1 and GIP, that help regulate appetite. In clinical trials, weight loss was reported when it was paired with diet and exercise. Results vary.

  1. 1Your stomachFuller for longerFood leaves your stomach more slowly, so you stay full for longer.
  2. 2Your appetiteQuieter hungerIt acts on the brain’s appetite centre, turning down the “I’m hungry” signal.
  3. 3Your blood sugarSteadier after mealsIt helps keep your blood sugar steadier after you eat.

Off-label use

What off-label means.

Mounjaro is approved in Japan for type 2 diabetes, not for weight loss. Before anything is prescribed, your doctor explains what off-label use means for you, including the risks.

Questions

Before you start.

Who can take it?

Adults 18 and over with a BMI of 27.5 or more, or 23 or more with a weight-related health risk, such as high blood pressure, type 2 diabetes or dyslipidaemia. A doctor decides: even with a high BMI, treatment may not be suitable.

Who shouldn’t take it?

It isn’t for anyone pregnant, breastfeeding or planning a pregnancy, anyone with type 1 diabetes or a history of pancreatitis, or anyone with a personal or family history of medullary thyroid cancer or MEN2. Tell your doctor about any allergies and every medicine you take.

Is it approved for weight loss in Japan?

No. Approved in Japan for type 2 diabetes. Prescribing it for weight loss is off-label use. Japan’s Relief System for Sufferers from Adverse Drug Reactions doesn’t cover off-label use. A medicine with the same active ingredient, Zepbound, is approved in Japan for obesity disease.

Can I take it if I’m on the pill?

Mounjaro can affect how well tablets are absorbed, so the pill may work less well. For 4 weeks after you start, and after each dose step-up, a back-up method such as condoms is recommended. Always tell your doctor you take the pill.

How do I inject it?

Once a week, on the same day, at any time. Inject under the skin of your stomach, thigh or upper arm, and change the spot each week. Each pen is one dose, and a step-by-step guide comes with it.

What happens after the first month?

The first-order set is 4 pens of 2.5mg, about a month, delivered once. After it, unless you change or cancel it in My Page, Mounjaro Trial Step 2 starts (3 months, 12 pens, ¥147,000). Your doctor decides whether, and when, to raise your dose.

What are the side effects?

The most common are nausea, diarrhoea and constipation, especially when you start or step up a dose, and they often settle. Get help straight away for severe stomach pain that doesn’t go away, which may be pancreatitis, or for signs of low blood sugar if you take diabetes medicine.

When am I charged?

Consults are free, first and follow-up. You pay for medicine only once a doctor prescribes it. If medicine isn’t right for you, you pay nothing.

Start low. Step up with your doctor.

A few questions to see if you’re eligible, then a free video consult.
